ALL MEDICATIONS MUST BE BROUGHT TO THE CLINIC. There will be no medication in the teacher’s room or on the child’s person, nor may the child give his/her own medication (certain exemptions may apply). Medication to be sent home will be placed in a sealed envelope and the parent must assume responsibility for this transport in any direction.
Prescription medication may be given at school if it is in the original pharmacy labeled container. No medication received in an envelope, foil, or mislabeled container will be given. Prescription medication must be accompanied by a note from the parent requesting the school to administer the medication, and specifying the time(s) and amount to be given.
THE ADMINISTRATION OF NON-PRESCRIPTIONMEDICATION IS NOT PERMITTED. Non prescription medication includes, but limited to: analgesics, antipyretics, antacids, antihistamines, decongestants, cough/cold preparations, throat lozenges, home remedies, homeopathic preparations, vitamins or other nutritional aids, and physicians samples.
It is required that a Medication Request Form be completed by the child’s physician and parent if medication is to be administer or available as needed on a long term basis (longer than 10 calendar days). The school nurse will provide this form.
At the end of the school year, medications will be sent home with the student in a sealed envelope. Parents must assume responsibility for this transportation method. If the parent desires any other method of medication disposition, notification must be made to the school nurse before the last day of classes. The nurse will attempt to make contact with a parent if a medication is left in the clinic. Any medication left at the end of the week after the last day will be destroyed.
