Our annual district Flu Vaccine Clinic will be October 9, our staff development/work day.

If you would like to participate, you will need a copy of insurance and license, no exceptions. The clinic will be held in the AHISD Boardroom. The registration and other information forms are attached for your review. If you are not able to complete the required forms in advance, paper copies of the necessary consents will be provided on the day of service.
